Higher responders to MiniMed Smart MDI alerts posted an 8.1pp higher time in range and 0.6pp lower HbA1c in a 143-patient trial, with EASD 2026 data reinforcing a real-world signal.
Type 1 diabetes patients who responded to more MiniMed Smart MDI alerts hit better glucose numbers in a six month trial. High responders averaged 8.1 percentage points more time in range (TIR). They also averaged 0.6 percentage points lower HbA1c than low responders. The data landed at EASD 2026 in Milan.
The MiniMed Smart MDI System Pulls InPen, CGM and the Go App Into One Flow. The MiniMed Smart MDI system links the InPen smart insulin pen with a continuous glucose monitor. The MiniMed Go app sits in the middle. The smart pen setup tracks doses. It runs dose calculations. It pushes missed-dose and high glucose alerts alongside glucose insights. MiniMed’s pitch is consolidation. The MiniMed Smart MDI system puts those jobs in one app. Users no longer have to coordinate a pen, a glucose monitor and dosing decisions in separate places.
The signal came from a six month randomised controlled trial of 143 type 1 diabetes patients.
Users with higher alert response rates averaged 8.1 percentage points more TIR. They also averaged 0.6 percentage points lower HbA1c than lower responders. That is a sizable gap for a behavioural lever rather than a drug or device swap.
A real world dataset covering over 5,500 users ran in parallel. That dataset showed a 9.4 percentage-point TIR gap between higher- and lower-alert responders. The real world figure ran slightly larger than the trial figure. Both pointed the same way.
Professor Laurent Crenier led the study. Crenier heads the endocrinology department at HUB – Hôpital Erasme in Brussels, Belgium. Crenier commented: “These findings show a clear association between responding to Smart MDI alerts and better glucose outcomes. For people on multiple daily injections, that can mean more time in range with less day-to-day mental burden.”
The findings were presented at the 62nd Annual Meeting of the European Association for the Study of Diabetes (EASD). EASD 2026 ran from 28 September to 2 October in Milan, Italy. Dexcom was among the other diabetes space firms presenting at EASD 2026. The CGM specialist showed findings from a report.
The MiniMed Smart MDI system secured FDA approval in January 2026. MiniMed itself was formerly Medtronic’s Diabetes business. It separated from the parent and completed a $560m IPO on the Nasdaq exchange in March 2026. In September, Medtronic launched an exchange offer. The offer lets Medtronic shareholders swap Medtronic ordinary shares for MiniMed common stock at a 7% discount. The move completes the separation.
Smart insulin pens, apps and CGMs are reshaping multiple daily injection care. The MiniMed Smart MDI system is one of the fuller stacks in that space. A clean association between responding to alerts and real glucose outcomes gives clinicians something concrete. It is a measurable lever on patient behaviour that already sits inside the device workflow. For diabetes clinics, endocrinology teams, and hospital procurement, the MiniMed Smart MDI system is one to track against Dexcom and the wider CGM-plus-pen field. Coverage on Medigear.uk follows the EASD 2026 readouts, FDA rollout data, and the MiniMed Medtronic split.
