University and college departments equipping teaching lanes, and practices hosting pre-registration placements, get substantially more educational value per pound from a manual head than from an automated one — and can equip several teaching stations for the price of one automated lane.
School and community screening is the manufacturer's stated second application, and the reasoning is sound for fixed screening rooms. An instrument needing no power, no charging and no network is straightforward to site and cannot be defeated by a flat battery or an absent socket. The practical qualification is that it still requires an arm or stand and an upright seated patient, so it suits a dedicated screening room rather than a genuinely mobile service.
High-street spectacle shops and hospital ophthalmology departments use manual heads both as primary instruments in secondary consulting rooms and as redundancy against automated instrument downtime. When an automated head goes away for service, a practice with a manual instrument continues refracting; a practice without one cancels appointments. At this capital cost, that insurance is inexpensive.
General community optometry uses the full auxiliary complement routinely — duochrome and binocular balance via the red-green and red lenses, phoria measurement by von Graefe using the dissociating prisms and by Maddox rod in both meridians, pinhole to separate refractive from pathological acuity loss, and the ±0.50D fixed cross cylinder for near addition. Practices should note that the 5° axis step limits precision on high cylinders, where trial frame confirmation remains advisable, and that patients unable to sit upright at the instrument need trial frame refraction regardless.
