The T606+2 belongs in musculoskeletal physiotherapy departments, sports medicine clinics, podiatry services and orthopaedic outpatient settings, wherever chronic tendon and fascia problems are treated in courses of several sessions. The conditions most often treated with radial shockwave are plantar fasciopathy, lateral elbow tendinopathy, Achilles tendinopathy, greater trochanteric pain syndrome and calcific tendinopathy of the shoulder.
The evidence deserves an honest reading. NICE found no major safety concerns with shockwave therapy for refractory plantar fasciitis and Achilles tendinopathy, but judged the efficacy evidence inconsistent and recommended special arrangements for clinical governance, consent, and audit or research. Its 2022 guidance on calcific shoulder tendinopathy, now republished as HealthTech guidance 645, goes further and recommends use only in research. Individual trials point both ways. A confirmatory multicentre trial of 245 patients with chronic plantar fasciitis reported significantly better pain, function and quality of life after three sessions of radial shockwave than after placebo. A double-blind Norwegian trial published in 2024 found that radial shockwave, sham shockwave and a standardised exercise programme, each added to advice and customised foot orthoses, produced no significant difference in activity pain at six months.
For a buyer, that points to a specific use: refractory cases that have already had a fair trial of exercise and load management, treated within a written protocol and audited outcome measures. The unit's shot counter and stepped pressure support exactly that kind of record-keeping. It suits pathways where patient selection is careful and the clinical team is prepared to measure results.
